Valued Westpac Labs Client and Care Provider,
Careviso is a third-party eligibility check provider who can submit prior authorizations on behalf of the providers. Providers who are not currently enrolled are asked to fill out and sign a BAA (Business Associate Agreement) which allows Careviso to submit the prior authorization claims to all patient's insurance on behalf of the providers. Once completed, please fax the document to the following number: 661.716.2187.
